If a student cancels, he or she will receive a full refund, minus a $50 processing fee. There are no cancellations after June 9. If UT cancels the camp for any reason, students will receive a full refund. The University may postpone or cancel this event due to circumstances or conditions beyond its control, such as, but not limited to, natural disasters, acts of war, acts of God, or public health emergencies. | Make checks payable to The University of Tennessee and mail application, deposit and parental permission form to: UTAD

I understand that should a medical emergency arise, UT will attempt to contact me, but that if I cannot be reached by telephone, I hereby authorize UT and its representative(s) or agent(s) to secure any necessary treatment, including the administration of anesthetics and surgery. I authorize any physician, healthcare provider or hospital to provide reasonable and necessary medical treatment or supplies. Other than a medical emergency, I authorize the University to examine and treat my child in the same way that University students are treated with the notification of parents being dependent on the judgment of the physician. I assume all risks associated with participating in this program, including informal extra curricular activities not specifically mentioned in the brochure such as games or sports, including bodily injury and death. I hereby waive and release UT, its employees, agents, and directors from all liability arising out of any sickness or injury, including death, that may occur while participating in this camp. |
